In accordance with the KCSOS Workplace Violence Prevention Policy, we are seeking employee input to develop and implement the plan. The following survey is designed to gain valuable insight about all KCSOS worksites and ensure our commitment to the safety and health of all employees.
Upon completion of this survey, your feedback will be sent to Human Resources. Your responses to this survey are anonymous. No employee will be subjected to termination, retaliation, or discrimination for making complaints, instituting proceedings, testifying with regards to employee workplace violence, or reporting a workplace violence incident.
