Report a work-related injury or illness

If an injury results in the need for urgent emergency medical care, call 911 and notify your supervisor immediately.

However, if emergency medical treatment is not required, injury reports need to be completed. If medical treatment is requested by the employee, Human Resources will make arrangements for the employee to be seen by a doctor.

After an Injury (non-emergency)

  1. Complete the Employee’s Report of Injury and provide it to your supervisor. (Forms below)
  2. Your supervisor will complete the Supervisor’s Report of Injury and have you contact our 24 hour nurse line at 1-855-812-2778.
  3. Employees are not required to seek medical care. You have the ability to seek care at a later time (up to a year), should you need to.
    1. If you are interested in seeking care after speaking to our 24 hour nurse line at 1-855-812-2778, they will make all the necessary arrangements for your appointment with a medical professional.
    2. For those not seeking immediate care, the Employee Report/Supervisor’s Report of Injury will be kept on file in Human Resources. If medical care is needed later, contact Human Resources at (661) 636-4747 or by email at hr@kern.org.

After hours Care
Employees can receive information and assistance with a workplace injury that occurs after hours by calling 1-855-812-2778.
