If medical treatment is requested by the employee, Human Resources will make arrangements for the employee to be seen by a doctor.<\/p>\n<p><strong>After an Injury (non-emergency)<\/strong><\/p>\n<ol>\n<li>Complete the Employee\u2019s Report of Injury and provide it to your supervisor. (Forms below)<\/li>\n<li>Your supervisor will complete the Supervisor\u2019s Report of Injury and have you contact our 24 hour nurse line at <strong>1-855-812-2778.<\/strong><\/li>\n<li>Employees are not required to seek medical care. You have the ability to seek care at a later time (up to a year), should you need to.\n<ol type=\"a\">\n<li>If you are interested in seeking care after speaking to our 24 hour nurse line at <strong>1-855-812-2778<\/strong>,  they will make all the necessary arrangements for your appointment with a medical professional.<\/li>\n<li>For those not seeking immediate care, the Employee Report\/Supervisor\u2019s Report of Injury will be kept on file in Human Resources.
